It's a little confusing the way it is now. There was talk about making the mouse cursor change to a wrench or something when you click the button, to hint that you need to point at something with it, but I guess it's not been added yet. ;p
Yeah the change of mouse would be good.
It changes when you point at vehicles, not when you click the button. That's what probably makes it confusing, because by pressing the button you don't get much of a visual indication that something has changed.
Personally I would suggest, in addition to making the cursor change when you press the repair button, making the icon a wrench only, without a pointer, and make the wrench a bit bigger. That way you can't mistake the cursor's current function to be anything but repair.
Anyways, that'd be my preference.
You are spot on. I saw the change in the mouse cursor after I found out how to repair.
The suggestion of changing the mouse as soon as you click repair would indeed help. That and maybe highlight the vehicles.
And/or add a "Repair All" button below it.
