Hit Without Missing achievement

Can anybody confirm where and how this is rewarded? Is it the ambush in Chapter 5 or the combat sequence in Chapter 4? I've done both a few times without missing and it does not trigger.

For anybody wondering, it's right after you shoot Lu Yue Ru.
You have to pick Tactics option (Chu Yao) and hit all three targets.
