Do favorite effects have any impact on gameplay? Or is it just for the hell of it?

short answer yes it matters.

otherwise;
it increases the odds for getting them as customer on free samples and for increase sales success % when you offer directly to them

but I think when they call for drugs they have a higer chance of asking for drugs with those effect in them.
