Thank you for the suggestion MiscOne. I don't use headphones. I am using Logitech 5.1 surround speakers and a Soundblaster Z sound card. Your suggestion did help me find the right solution.
To fix the audio issue you have to make sure your sound/speakers are set to Stereo Direct or Stereo 2.0/2.1. If you have the audio set to 5.1 or above it causes the sound stutter issue.
I'm not sure why Final Fantasy III has not given me this issue. I can run the mkv file that contains the opening movie for IV just fine on my computer; however, when the game tries to play it gets super choppy.
Again, thanks MiscOne, you suggestion helped me track down a fix.
Just tried this, and yeah, my audio device was set to "7.1 Surround" by default (I'm using headphones). Switched it to Stereo and the intro cinematic sound is perfectly fine now.

There are audio issues if you do not set your speakers to 2.0/2.1. The main ways you will see the FF3 audio issue is as follows:
1) Sometimes you will have no overworld audio. The same can happen in dungeons.
2) You will never hear music play when you are in a vehicle including Chocobos. You will hear sound effects, but no music

I am also using onboard sound that was set to stereo.
This 'Spatial sound' setting was Off but, for me at least, there is also another one right above it called 'Audio enhancements' which turning Off solved this issue for me.
Disabling both 'Spatial sound' and 'Audio enhancements' was what worked for me also
