Also, I assume you mean the platform above and across from it, or the one directly above?

The tricky part is the jump, go to the outermost rim of the platform and jump at an inward angle.

Hopefully that helps. Its been awhile since I touched the game.
1) Use AOE to destroy all the candles.
2) Make sure you've taken off the winged boots.
3) Run along the inside (of the circle), shorter edge (where the candles were).
4) When jumping off, keep holding the right button/direction.
Once I did it that way, got it in 1 try.
Note: I've read elsewhere that the frame rate setting might be partially at fault for this. I'm not sure if that is true, but I didn't care to go futzing around with settings, and I don't think that's the real issue here.
The solution is simple--use a controller for this jump, and do it after you get the Shadow Axe in the Time Crevice (the Veda's Gift passive it gives you increases your movement speed). Reason being, when using a controller, your momentum is preserved, whereas using mouse movement, for some reason, it is not. Combining this with the movement speed boost from Veda's Gift makes the jump absolutely trivial (I made it easily on my first try doing it this way).
Here are the exact steps I followed, if you want to reproduce them.
1) Get to the ledge you need to jump from and switch to your controller (you may need to restart the game with your controller on for it to work--I did)
2) Remove the boots
3) Use AOE magic, like Ignition, to kill all the candlesticks (might not be necessary, but doesn't hurt to make sure)
4) Equip the Shadow Axe and its Veda's Gift passive
5) Do the jump and open the chest to get Gray Ash
