I *think* it has to do with your influence score, or possibly renown, but no actual idea. It's another one of those things that happens so late, you can't really mess around to test the parameters.
Maybe replacing Abdul is not the wisest thing, can even sympathise with his corrupt stuff.
I can further confirm another option: wife's resentment. Just pull up the event to resolve the resentment and add the Sultan's suspicion. Your wife will beat you up and leave you for 6 days (she returns afterwards). I think you also get a wound. Pretty cheap option and it is only a ruse after all (might be different if you have multiple resentments or other issues with your wife).
Another option is either the feast or wife's tea party quest. I haven't tried that one and don't exactly remember which one it was (I think it was the feast). I saw that you can add Sultan's suspicion to one of those, but haven't tried it out yet.
Finally, when I have used it in court, the effects had been different than Gilias described. I didn't get a Sultan quest but rather lost 5 influence points. Might have something to do with me not personally presenting the card in court but rather sending a follower (usually Farad).
Interesting note: If you are unable to save her and let her die...Sultan, regardless live or dead, will release all his fury on you, by giving you another group of Suspicions, they range from 5-8 cards. I usually get these after I kill Sultan and enthrone myself.
One other option is to limit your disagreement with Abdul and keeping your influence as low as possible, so that you will get very limited Suspicions - the only bonus you lose is the support cards, which you rarely need if you manage rebellion well, and you automatically get another set if you are friends with that certain someone.
If you never go to Abdul's request, there is also a special bonus - you get a poetry book every time you attend meetings, which can be read to add battle points. So maybe that option is better for those who does not care about Abdul, lol.
If any of you are troubled by Suspicions...my record is 17 of them in 3 bunches: The highest record in 1 time is 8 cards bombing my inbox. After that run I am convinced if I don't become the next Sultan, no one can do the job.
