Using it has a chance of clearing the next room of enemies. If this happens, a letter will be left behind.

From my experience, it is guaranteed. Used it 3 times in a row on the 5th floor and all 3 were cleared. Same thing happened the next run except on the 4th floor. You wont get a drop/money, so I would only recommend doing it if you already have a decent build going or just don't want to risk any hp on the final floor while you are looking for the boss.
