Health Upgrades
I'm fairly new to this game and want to make sure I'm getting these health upgrades right. When you flawless a boss, you gain a heart container, but it's not a permanent upgrade? So I have to get a flawless on every run if I want to increase health?

Well it is permanent for that run only.. in that you can lose the heart and refill it during that game. But yeah, you die all the way you start with the beginner health again.

That said, there are other items (passive items) you find randomly that give you extra heart slots as well, similar to binding of isaac (tho not as frequent). Not to mention stronger items you will be unlocking through the gun shop, new NPCs who might help (a guy that sometimes comes at shops to offer more stuff, the elevator repairman, and the like), so there is a little bit of persistence throughout.
