ETGMOD.zip, what is this??

Installer for Mod the Gungeon comes as executable, *.exe file,

https://github.com/ModTheGungeon/ETGMod.Installer/releases/download/v19.04.12.2/ETGMod.Installer.exe

Here you have recent version, after launching you have to provide full path to Etg,exe, main file launching Enter the Gungeon, so in your case this should be: 'D:\Program Files (x86)\Steam\steamapps\common\Enter the Gungeon\EtG.exe, or wherever you installed the game. Then you click icon, and everything should install fine. If the installer won't launch, make sure you have .NET Framework 4.5+ libraries installed.

When done, you put your mods, as a zip file, in the EtG/mods folder, don't unpack it or nothing. (in mods.txt file you can specify which mods you want to load, but it's optional).
