What is the best Active Item?
I'd say Spice. It singlehandedly carried my currently best run; I killed the Old King and reached the High Dragun, both being my first time doing either.

Only downsides is the health container loss (which isn't much of a downside when you have tons of empty containers that you can never fill up anyway) and it becomes incredibly common after using it for the first time, replacing merchant items and room clear drops.

i'd say the potion of lead skin. its essentially stuffed star+rolling eye.
just stand in front of bullet hell-y enemies and they'll do tons of self damage.
very good for the lich fight.

Potion of lead skin. This thing saved my you know what countless number of times. Too bad it's a "D" item which makes it rare in my book, because I almost never open brown chests.

I'll go with Potion of Lead Skin. Very useful, appears often, decent recharge time. Potion of Gun Friendship is very nice too.

You overused spice; the first two uses are completely beneficial (well minus the curse) while the third onwards adds damage, etc. at the cost of health slots.

I use it twice and that's it; health is usually the most valuable resource in a run, and a slight buff to weapons isn't usually worth it. The fact it starts replacing your item pool by like 90% after 3 uses also is another reason to not go past 2.

Cigarettes early in a run are a game-changer, other than that, any invulnerability item (especially the ethereal ring) is great for utility and overall use.

Reason I used it past the 2nd mark is becuase I had tons of empty heart containers that I could never fill up. Picture yourself having like 8 heart containers, but you only have enough health to fill 3 and never get any of it back. For this very reason, the only real downside to using it was the curse. I ended up taking around 6 - 8 hits in total.
