'nuff said.
Shotgrubs are an honorable mention only because they're easy to kill at this late in the game. I usually end up rolling towards them when they fire. The bullets that hit the walls way behind me usually pose little threat.
The Lead Maidens in Bullet Hell are still 5 times as worse.
I can think of a couple. The one with the checkerboard pits?
I think I remember fighting 2 at once as well in a different room.
Then there are those gold sheleton summoning lich dudes who aren't bad on their own, but if you get them in a massive room with little cover AND one of those other wizards that speed everyone else up AND you get challenge mode so everyone is invincible except the sheleton summoner lich dude who runs away from you faster than a freakin' lightning bolt cuz h e has the speed-up buff. ugh.
As it turns out, they make excellent flanks and surround me easily, not to mention that they make boss fights absolutely horrible.
After that, perhaps the bullat. Why?
Sometimes they fire once and sometimes they fire three f**king times and there is no way to tell beforehand which it will be. You only get a very brief audio notification on the repeated shots which is fine by itself but when there are other bullats it is easy to miss.
Honourable mentions go to Lore Gunjuror and Coalers though.

Honourable mention definently goes to Shotgrub, those things make my run literal hell, but they are usually on their own and don't really have enough health to make a true difference in the run.
