You have to place the ACC.exe file from the folder with your resolution - in the main directory of the game and replace the original ACC.exe. [backup your original .exe before replacing it because this can mess up your game if the patch is buggy]
I say "if the patch is buggy" because the first version of it that i used long time ago worked like charm, but when i downloaded it couple of months later - it did made my resolution ultrawide, but it came with a bunch of bugs (like the leaderboard not being able to connect to uplay), and alos slowed the game with couple fps. The new version i uploaded in the link from my previous (edited) comment appears to not do this for me anymore. Also i have no idea why the previous version caused this problem, but this current one from my new link works without any problems.

Also later i checked the Black Flag wsgf page and there it said that the links were moved to the pcgamingwiki and so i searched for the pcgamingwiki page of AC Rogue and there were also links for different resolutions including 21:9 UltraWide:

The files inside the archive from the two wsgf links seem to be different size for the same resolution and also the second link has 2 versions for each fix, V1 with normal FOV and V2 zoomed out and with much wider FOV (you can see the difference by clicking on Spoiler: Show). I don't know what is the difference between the file from the first link and V1 file from the second link, so you might want to try them both and use the one that works best for you if there is any difference you can see or experience.

Best get it before that one stops working. Keep it somewhere on your pc or re-upload if anyone can be bothered.
