To me he just always looks out-of-focus. Maybe it's made that way.
Also, you should turn off SSAO. It adds an ugly-looking black aura around all objects in the game and it's especially noticeable on Snake.
You know that during the snake eater mission big boss code name was Naked SNAKE and after that he asked people to keep calling him Snake right????

I think that the "High" settings are the same effects used for the PS4 version. I would argue that these settings look much better. A less blurry snake.

Yeah but he's blurred in a dumb, crappy looking way. It's clearly not working as intended, and perhaps due to the Bokeh filter which is only present in the PC version AFAIK
