I haven't seen a difference to tape collection depending on the difficulty.
I also think that it's not necessary to complete the mission after recovery (Edit: but the mission will need to be completed if it's the last tape needed for Chico's Tape (All), which is unlockable only through the post-mission rewards screen), just that there is a checkpoint reached after acquisition so that the game saves. You can test this out by playing the GZ mission and going for Tape 5 and/or 2, then running to get Paz or Chico to ensure a checkpoint occurs and finally exiting the mission. I was missing Tape 5 and did this successfully (count went from 16/19 to 17/19 and the tape is in the library). If completing the mission was necessary for correctly reading to get the achievement, but this is allowable, my count would be 18/19, since I have the full library.
It seems that our only way of resolving it so far would be to run to the physical location of the tape in each appropriate mission and see if it's still there, despite passing the game's check for it in the library. Also do the missions that give a tape upon completion again. We might be able to bypass this cause I heard somewhere else that running the Classified Intel Acquisition mission and holding up the guard for the secondary tape and picking it up will make the game recheck the tape library, which might read our full collection in the library. Then someone else apparently had to play EACH tape in their library for it to register, but I already tried this myself to no avail.
Afraid if all of them are in the library, you'll have to just manually check all the tape locations.
Apparently, what might have happened to us was that we had gathered all the Chico tapes, but then exitted the mission before completion, which omitted us from getting the Chico Tape (all), since it's only retrievable through the rewards popup and not automatically given upon registering all 7 tapes.
I went into the GZ mission and extracted Chico, before Paz, at the mission start extraction zone (giving Tape 3 again); incase that somehow factors into it.
Another thing you could also try in the name of science is another mission entirely that you can do really fast and then check the rewards screen after completion to see if it will trigger the Chico Tape (All) flag. This would mean the game checks the player's tape library whenever the rewards screen is scheduled to show. If all seven tapes are in possession, the game unlocks the Chico Tape (all).
