2. Prone is important. Jumping into prone is simply cool, and an effective way to dodge gunfire and get behind cover if you need to.
3. There is. On a controller you click in the left analog stick and your character will start running for his or her life.

5. I don't think so. I just whip my binoculars out, point blank at a wall, and tap the crouch/prone button to pop up (A on controller), see what I need, then pop back down.
There is a way to peek out from cover, but not with binoculars. To peek from cover, press the zoom button. Right thumbstick with controllers, V with keyboard/mouse.
I have to press onto the cover in order to stick to it. can also easily get out if just by moving away.
The "sprint out of there once spotted" thing is not really a thing.
Once reflex mode engages(everything around player slows down), the player is meant to disable the soldier or w/e found him out while the reflex mode is in effect. By w/e means available. Running away will just trigger an alarm, which is not the point. We want to avoid alarms being triggered.
Oh, there is a "gtfout of the way" button. For console it's the square. Which is very handy in case you are about to be spotted. Snake will throw himself in the direction the player chooses. Can also go into a dive after running. Like I said before, this is not effective one you trigger Reflex mode. (aka, just got spotted) Best course of action is to eliminate the threat.
I recommend looking in the key bindings and going through the game's options to check out all that you can do and stuff you can change to your liking. Also experiment with his moves to find out all you can do. Sadly, no tutorial all thanks to the nature of this release.
Unfortunately, it doesn't look like TPP will solve this problem, but hopefully the automatic cover system is a bit more solid.

Holding the A button or the stance button on PC will take you to prone in the spot you're currently standing. This is good if you want to crouch or lie prone behind something beside you. The other button you're mentioning is the dive. The purpose of the dive is not to move you into prone. It's a replacement for the roll command and it's supposed to give you some extra range of motion when in combat or stealth. You can dive behind cover when moving under fire or sneaking around a guard to limit your exposure. In one case you take less damage, the other you may keep from being spotted and raising an alarm. It's actually one of the most useful actions in the game.
Press A or the stance button once while prone and then click the left analog stick or press the sprint button on PC.
Click the right analog stick or press the camera button on PC to switch camera sides and have a better look from behind cover.
What you want to do can only be done manually, but it's incredibly risky to do so with enemies around. However, I don't see why you would want to use your binoculars to spot enemies close by. Instead, use the peek action to mark enemies that are close by from behind cover. If there are no enemies nearby, just use the binoculars to spot things from a distance and don't worry about being spotted.
FYI to sprint using a controller you don't have to hold down the left stick, just tap it once.
I went for 100% using a controller since I had previous experience playing this game on both Xbox 360 and the PS4. It's the most complete input method, you can tell the game was designed for it. But the precision of the mouse when shooting is ridiculous. I started playing solely using the Keyboard & Mouse and there are times when I feel like I'm cheating by using the mouse.
There are some issues with using the K&B, though. Guards can't be let go from a hold, aiming with a scope is horrible since the moviment grid doesn't scale and other things. Hopefully fixed when TPP comes out.
Yeah, it's actually the same with PC. I hadn't realized it at the time of posting and kept holding it in regardless.
