How do I unlock this fragment?

I've played through and completed every fragment for each team except the second from the top on the left side for the Q team. I've gotten all four execution outcomes as well, and there are no exclamation points on the flow, but this fragment remains locked. On my global flow chart, locked segments are the one below radical--6 and the one below suppression. Can someone help me with this?

If you post your flowchart it'll be much easier to understand what's the problem.

Usually if you can't get what you missed it's better to open flowchart for every fragment (not the global one!!!) and check if you can get to the unseen parts.

If I remember correctly this fragment is opened after Q execution scene... (this fragmanet is called Reality or - story spoiler - the talk between Zero and the boy where you should get helmet pass). Are you sure you have seen all execution scenes? They aren't shower scenes, they happen after voting and usually people miss at least one of them after they have seen all voting scenes.

NovaZeal, well... don't look for answers you shouldn't know^^" This game is too plot-heavy to use answers from google - you'll miss too much that way.

Under "don't inject" it is D-team fragment "Outbreak" - third on the right side. And as you guessed it's Q-team piece under Suppression. I believe you haven't seen D-team part but it's unlocked for you already so check it out.
