Completely stuck (Near end game spoilers)

So I've done everything I could on the fragments I have, yet I'm at a point where I need information to progress in the fragments and I'm at a complete loss of where to find those.

To try to give a vague idea of where I am, I've unlocked three endings (CQD1, D1, Q1) and I have 3 unknown fragments in C Team, 2 in Q Team and 1 in D Team.
The points where I'm stuck are at:

• Q Team's 3-way standoff: I didn't unlock the 4th last choice, I assume you need Q's real name. I learned he was modelled on a boy named Sean but it's just his model. I tried typing it regardless and yep its an error.
  
• Q needs to tell who's Mira's murderer in order to avoid Eric killing you. I don't think I've ever seen who killed her, unless it's implied somewhere else and I missed the whole info.
  
• Door of Truth: I just don't know the password
  
• When every team is saved and they need to something with the Force Quit Box or for Q Team: unlocking the access to the Quantum Computer. I don't ever remember Diana telling us about her mom's memento but I suspect it's the one I should have already the info.

Could anyone please give a hint on what to do next?

You're speaking about the Pantry, right? You don't need to really figure out who killed Junpei. You'll learn later in the game.

The C-Team and D-Team execuition buttons aren't lit up. You need to re-do the voting and see them die to open new fragments.

done it dozen time i think, maybe bugged im doing from the begiining of the vote, still dosnt lit up

You made sure to re-do the execution votes from the very begining of their fragments, right? Just chosing the outcome wont actually change what is being voted, the game kind've remembers what you had each team legit vote.

as i mention before, i redid everything since the coin toss until all the vote, still not lit

Rinto, game remembers your last choices in voting. Use the fragment with scales and watch cinematic after it until the end. You need to play as Q and D teams both voting for C-team and then play as C and Q teams voting for D team. You don't need to play from the fragment start. It's ok to start from the scales part, but you need to see it until you will be out on save/flow window.
