4k smooth gameplay with these settings
Specs:
I7 4790k @ 4.5Ghz
8 GB Ram
SSD
GTX 970 SLI
Windows 7
Newest drivers and newest patch as of 11/21/2015
60" Samsung 4k tv

Ok, so I have mine running much smoother now and thought I would share incase it can help anyone else. The game looks much better in 4k with Ultra textures.I was getting bad frame drops during campaign and MP. Meaning, I could stand in a closet and look at the wall so not much graphically would be going on and it would sit at 60fps for a few seconds then hit 42 for about one second then back to 60 for a few seconds then back to 42 for one seconds and would continue doing this causing stuttering. Now it sits at 56-58fps with minor stuttering every once in awhile. Smooth 95% of the time. I just restarted the campaign and played through the first 3 or so missions with these settings.

1: Page file unchanged ( I think its set around 12gb)
2: Config.ini fraction of video memory usage to target set at .9 ( I tried .75 and default .95 and couldn't tell any difference.)
---Game settings---
3: Display mode = Full screen
4: Monitor = 1
5: Screen Resolution = 3840x2160 (also tried 4096x2160 and stutters came back a little)
6: Refresh Rate = 59.940hz ( I tried 60 and stutters came back a little)
7: Render Resolution = 90% - 2456x1944 ( 100% also works but it felt more responsive at 90% even though the fps was the same.)
8: Field of View = 100
9: Max Frames Per Second = 55 ( anything above that caused more screen tearing)
10: Sync Every Frame = off (turning it on cures the screen tearing but has input lag that makes the game feel like it's running 10fps slower than it is.)
11: Texture Quality = Extra
12: Texture Filtering = High
13: Mesh Quality = High
14: Shadow Map Quality = High ( Extra caused stuttering )
15: Dynamic Shadows = On
16: Subsurface Scattering = On
17: Order Independent Transparency {OIT} = None ( anything else causes major stuttering )
18: Volumetric Lighting = Off ( Turns off automatically with OIT set to None )
19: Anti-Aliasing = None ( Using SLI we don't have any good options for it that don't make everything look too soft or blurry to me )
20: Ambient Occlusion = Enabled
21: Motion Blur = Dynamic

** Frame drop seemed to mainly be caused by Shadow map being on Extra and OIT set to anything except None, this will probably apply to other people having frame drop problems at different resolutions**

This is just what works for me and my setup, hopefully it can help others but it might not.

---Issues I'm still having---
1: Screen tearing ( If anyone comes up with a solution please post it )
2: Some textrues turn into bright lights sometimes. I see this more in MP.

Guessing you're running it via HDMI ? If so your HDMI out from your GPU might not support anything over 30fps @4k so cap it @30.

Yes HDMI. It is supported. This game would not play well at 30fps, too fast passed.

Just played through the mission with the storm and it ran at my frame cap the entire time. Also played MP witht these settings and checked vram. It was just over 3gb over the course of 4 maps. Working very well. :)

No, as I said. 60 seems to cause a little stuttering for some reason. And setting my frame cap closer to 60 causes more screen tearing. It runs nice and smooth at 56fps. The game has some issues, these are the work arounds that worked for me after a lot of trial and error.

everything on the highest settings except 3 things
AA is off (4k does not need that)
shadows on medium i think'
and
some lighting something one is on medium as well

edit; this is also on MP not SP, never played SP
