Bo3 has native PS4 Controller Support so it will show the buttons correctly.
Only for wireless. If you use a wire you dont need anything it'll work and im fairly sure with the correct buttons showing up.
No, even plugging it in requires you to set up a profile in DS4 or the computer won't recognise it. But I've already done all that in the past, I only wanted to know about the in-game controls.
Install InputMapper it contains required drivers for the controller.
Then you can choose either plug it directly with usb cable, the controller should change from flashing orange to solid blue when you lauch BO3, the controls and buttons will be like on PS.
Or use it wired with InputMapper running or wirelessly with InputMapper running to get XBOX layout.
I play with DS4 wired.

Yeah, I play it wired anyway since I'm still sitting at my computer desk and don't need it to reach further away.
