Question about the death machine in zombies

I was playing Origins just now and I only got it as drop a single time (if I remember correctly, if not I didn't get it at all), after I settled down and camped with the ice staff I never, and I mean never, got it as drop again. I know that with carpenter and fire sale you can manipulate the game to remove them from the drop cycle. For carpenter at least a couple windows need to be completely open for it to spawn and for fire sale the mystery box had to be moved beforehand. I'd really like to know if this was just a one time bug or if you can actually stop the death machine from spawning completely.

I looked online and people are saying both quick revive and turning the power on make it spawn. I didn't buy quick revive, but I had the power on, before losing 3 gens to those special zombies.
I played Origins a couple of times now with the camping strat and usually always got death machine drops, no matter the state of the generators, I also never purchase quick revive in my games.
