While playing CPU is at 80%, GPU at 99%, RAM 6.5 GB/ 8 GB usage.
I don't know, if the FPS doesn't drop, idk how it can not look smooth other than stutters, which often are caused by those options.
But I do know that it could be just an issue with BO3, since I myself had issues too.
Here you can see another issue: when starting a game even in main menu it stutters like hell and I get short freezes. When starting the first match it looks like this. The real problem with high fps but no smooth gameplay you will be able to see in the next video, I will inform you with link when uploaded.
I think but am not sure if I entirely fixed those, but I'm gonna play moon now, so I will see.

you cpu is good and should be more then enough but a lot of people got problems with i5`s and bo3. this problem was "fixed" long time ago but from time to time there people like you who got the same problem.
i dont have an awnser how to fix but maybe you will find something if you search for i5 and high cpu load
Turn off dynamic shadows.
Dont use sync every frame.
Your issue isn't your i5 and even if you got i7 4gen, you wouldn't see some big performance. Anyone claiming that is full of sh*t and have no clue how gaming works.
Nvidia drivers aren't the issue. Ive used several of them this year, not a single issue.
Now if you install all the crap that comes with the driver, those can cause some issues. All you ever need is the driver and PhysX.
In the nvidia control panel, you can set power management to maximum and set physx to gpu. This can help to a degree, it will depend on game and your video card.
Dont use any dvr or monitoring software. Make sure no other programs are running on startup or in the background.
