This is bad. I have a friend with a disability, so he can only control with a mouse, but he enjoys playing vampire survivors. I see more and more similar games are abandoning this control, I think this is not the right direction.

I had 3 games like this where after i asked the Dev added such controls. I think most reallyy want to help, they just don't realize that there's ppl like us. Which is understandable.

Can they play with a gamepad? There are even accessible single handed gamepads out there. If not you can use Joy2Key (old versions are free) to map xinputs to the mouse but not sure if you can map it to hold the direction.
An actual xinput joystick would work pretty well if they can hold that.

The game has a setting for mouse only.
Movement is handled by character moving always towards cursor and door opening is left click on door.

Only Mouse and only Left click (Macros and Script Weak scenarios)
