Health Potions not getting used?

Most of the time they don't even get used, so what is the point of them? And what triggers them? Because low health alone doesn't trigger them.

I had a run with the potion accessory that granted invuln to # hits = potion usage where it definitely was not registering the potions/giving invuln on hits - but then my last run with that accessory it worked flawlessly
