Severe Ironman Bug

I played this game on hard difficulty, once I was looking for a challenge so I set up Ironman mode (I had finished 3 scenarios and this was my fourth one). When I died, rather than restarting the scenario, it restarted the whole game. I currently have to start from scenario 1 and the scenario I was in is now locked. Has this happened to anyone else?
