although this event was also intended to stress that his inner will could defy the limits of his body and probably fate itself since the same happened with iwanako, I also think it was not necessary since he's the only one to blame if he keeps forgetting taking his meds and not exercising at all (that's why emi is my fav route: by discipline and perseverance you can certainly change your fate, not just by a sudden epiphany or by the magic of love). On the other hand, he also mentions that his lifespan was shortened due to this rush to the airport so not everything was positive...
Regarding the other points, I think lily's plot is not that "out of place" or intentional manipulation for several reasons:
*although Lilly portrays a mature/calm facade, deep down she was scared to lose her life in yamaku, but similar to hisao, she's reluctant to share her problems and worry / put a burden on others (hisao mentions multiple times her pride and discomfort to admit her shortcomings)
*even when her family abandoned her, the fact that her family was finally willing to have her around was still a difficult decision for her (during the dinner scene, hisao is surprised that she doesn't hold a grudge nor resent them, and if akira - her closest relative - was also moving, how could she abandon her when akira stayed with her when she was 12?). during hisao's confrontation, she initially mentioned something around "wishing to have more time to think about it", but...
*when hisao asks lilly about her decision in the shangai, hisao's first reaction is to assume that she was hiding it because she already made the decision to leave, which she ends up accepting for pride and hisao yells at her iirc, but calms down almost right away to emulate her good manners (btw his fake calmness and acceptance of "ok so when will you leave?" and "wish you the best" were a passive-aggressive way to let her know that she didn't mean anything to him neither)
all in all, I liked this path, the only thing that bothers me is the decision tree: based on the plot, hisao was able to positively reflect on his actions and advance his relationship with lilly by deciding to open up his thoughts with yuuko, but the decision points are at other random places that don't even affect the h scenes
