Hisao's first heart attack

How often in the routes does Hisao talk about his reason for his first heart attack with the girls? I only recall him doing it with Hanako but not the other girls. Granted, I haven't played Shizune's route on the steam release yet but even back in the day when I played the game I didn't recall him ever talking about his heart attack with her.

I think he only tells Shizune about his second attack, on the second path to her route. As I remember, this is quite uncommon choice scene, before which you had to choose "Yes" for "Go for it?", and also be on the Student Council. (and btw, if you choose to tell her about the attack, you'll ruin the way to her route)
