Revenge increases when hit by anything even if it doesn't do damage as long as its by a player.
There's also a feat that increases revenge as long as you're locked on.
I believe revenge increase is the same for all characters with no difference.
However I've seen people get out of revenge, literally dodge on of my attacks and then immediately get revenge, and that particular case was not a one time only thing, I've seen it happen multiple times
Or the exact opposite, a buddy of mine was getting tanked by the entire enemy team, he managed to parry, block or dodge almost every attack he had been hit a bit as he was half health, but he never got revenge like at all until he was one hit from dying
Like, 1 enemy, revenge gains little.
2 enemies, gains faster.
3 enemies, very fast.

Also, unblockable bash feeds a lot of revenge, beware using it when ganking.
revenge is a special SJW handicap for folks that can't play any other way.
