Hi thanks for reply. i also use one finger for trigger. i dunno why the rest can place both index and middle finger comfortably at trigger buttons. I also did try your way using one finger to push both RB and RT i know it works. yeah it works. just twist my right hand alittle in anti-closewise direction, index finger tip hit RT and knuckle RB. but it still feel uncomfortable for me. i am from singapore. asian hands not so big as westerner.
below is an example of how i hold my controller. **The below image is not me not my hand!

I live in singapore no walmat. rb and rt has to press at one go. is either i follow your way by moving index up and middle to the trigger which is very troublesome and slow. or i use just index finger tip hit RT and knuckle RB but is uncomfortable.
I am so tempt to buy razer wildcat or xbox elite
our middle finger is below the controller

Is your hand incredibly small then? I had no trouble putting my middle finger on RT :o
is probably different way of holding. i can move my middle finger up when necessary if need to zone attack. but troublesome and uncomfortable.
