Coop dual executions

I understand how a double execution works for me as an individual, but I'm wondering how to set up a dual execution between my coop friend and me. It's happened several times, but by accident. We can't seem to figure out how to deliberately do it. Anybody out there figure it out yet?

I think the prompt just occurs when you're in combat while close to your partner.

It comes down to the timing, so if you are both close to the enemy and try an execution at the same time, it should be a Co-Op execution.
