Why only 3 lives?
I've lost count of the number of them I've left behind on other levels....

I think you're supposed to be going through them at a rate of knots.

I haven't used one yet, it would good for people like us who keep them to be able to collect a giant stack of them. Could even be a challenge tbh
With high amount of armor drop and health recover with weapon upgrade ? They really mean nothing .
On highest diff in which people die due to being careless and greedy , they would choose to reload the game instead .
Because YOLO feels old.

Playing on nightmare 120% game speed, I've had more than 1 like, a couple of times.
It's kind of a buffer for the checkpoint system, which is really really bad IMO

Because the checkpoint system on open maps can save your location before 2 big fights back to back forcing you to get all the way back to the arena and fight again on failure. Automated Checkpoint systems just don't work for open-world games for this exact reason, worst case scenario it backs you up before several challenges OR it backs you up in a terrible situation. Best case scenario it will save often in between fights or triggers. This doom is a lot more "worst-case" than "best-case"

agreed on the last sentence, but everything else i don't remember witnessing. but still sucks that it happened to you and others

it's mostly a problem on nightmare difficulty with tweaked harder settings because you will die a LOT more than the devs probably intended.

Because in Eternal, you could exploit this mechanic by constantly reloading checkpoints, which in turn allows the "Extra Life" icon to respawn.

I remember once I played it that was and stocked up to 99 Extra Lives (99 is the maximum) without even finishing the 1st level.

The Dark Ages is not going to let you cheese like that.

Issue with 2016 and Eternal is you can just seemingly collect them forever and if you got into a tough fight you could just brute force it with as many lives as needed. Capping it makes it so you need to decide when it's worth using it. Whereas in the previous titles it would just automatically use it for you. Here you can decide if it's really worth it or if you just need to take another swing at the fight from a different approach.
