what is required to get a perfect run and how does a companion mouser kill you?

You have to enter a Portal of Mice to get the Mousers to explode on death. When a friendly one is killed by an enemy, you need to be in the explosion radius with very low health and it will kill you.

I do not know for certain, but I am assuming a Perfect Run is taking every shimmering portal and gauntlet challenge offered.
