It's one of those items that are contraband sold by other inmates. Keep checking the guys with a coin icon. As an alternative, you can use a file to take down a vent but this also destroys the vent crate itself (as opposed to taking it off and having it in your inventory and being able to replace it and cover the vent shute), so you might want to craft a fake vent cover out of paper mache.

Im not the only one who has been ♥♥♥♥♥♥ over by a fake vent cover breaking so always have a spare lol.
