Iron titan achievement

I need help figuring out this achievement. It says 'Beat the game in Iron mode with NG+'. What does that mean? Isn't NG+ after beating the Truth? When all titans are available to be killed infinitely?
My only guess would be that, if I continue my iron mode playthrough, and beat the truth, I'll go back to the begining and then I could go straight for the soul (again) and get the achievement, but that doesn't make much sense. Is that it? Or how does it work?

for this achievement, you need to start the game in hard mode+iron mode and pass it, a very difficult achievement for those who play hard mode for the first time, because it will seem that it is impossible to pass it without dying.
If you want to test your nerves, then you just need to remove 11 bosses, go to the second gate and defeat the last 2 bosses there.Iron Mode is a mod in which if you die, you will start from the beginning, and it's still hell in Hard Mode, NG+ it's Hard Mode

Thanks, the 'NG+' instead of 'hard mode' confused me a little.
Got the achievement last night, and today I just got titan god, tough runs but it's finally done
