Achievement glitch and game crashing

Ever since the game released, I have been unable to get the MAINTENANCE EXPERT (swap 100 fuses) and ONE MORE? (die 10 times). I have 6 hours of playtime and beat it on the first try, the rest of it has been grinding for those two achievements but they wont unlock. Also, every time I hit continue game and get to the part where you have to click on the center monitor where the tips are shown, the game crashes and I get an unreal engine crash screen. Any ideas?

For the crash, Nikson is still trying to find out what is causing is, but he added a setting to disable tips, and switching to DirectX11 when you start the game should also fix the issue.
As for the achievements, I don't know, I have been able to get them without any problem.
