It is possible to clear a POI of zombies, and its possible the POI is already clear of zombies without player involvement (in my experience this happens at the Radio Station quite a bit). There are times where places like the Field Hospital are devoid of attacking zombies but have a few lingering inside, other times its overrun and keeps being overran. Same happens at other POIs, like the Sluice gate being seemingly empty or the largest endless runner horde imaginable.
Stealth does appear to play role to an extent, but really, if POI is overrun then its very likely the zombies are continually aware of your presence at the POI, but not always as to where exactly unless in view or you are engaging in combat. Since they flock around objectives in POIs, you should generally treat it as "Stealth not being an option" in those cases. Retreat and gear up as needed, or band together with others.
(Nightmare difficulty is a completely different beast and you should just expect the worst at all times)
