2. Die from any other cause than getting killed by a monster, infection process completing, headcrab jumps on you.
3. As a headcrab you can evolve but have to be far away from others.
On the lobby settings was the "Never Spawn Playable Enemy?" option enabled?
If you die from Infection strangely you do not become a monster but rather yo die and your corpse walks around as a npc attacking people {this is so silly}
lots of lobbies doable playable monsters in my experience!
