How to be a playable monsters?

I've heard you can sometimes be a playable monster after succumbing to infection, but it's never happened. My character just t-poses in place. How do you become a playable monster?

If your game is set to the custom setting to allow for dead players to become a monster upon spectate, you should automatically after a couple seconds of being dead, get spawned in as the base form of the monster.
