Of course, it you're running around at night, you're not likely going to hear them over your own footsteps.
However, if you're standing still, you can always hear their footsteps and they'll often screech as well.
At night, I get inside a shelter, sit in front of the door, and I wait...
If anything comes through the door, it's getting shot.
2) In game music really tells you about monsters, put it at 100% sounds, music, all of it.
3) AI Monsters make a lot of noise+music, easy to spot them.
i legit was in a match with a friend and we were sitting still inside of a tent in the medical place
the only reason i realized a monster had gotten in through the closed doors was because my friend started shooting at it
like i said, for me these things are pretty much entirely silent, and i'm not sure if that's a bug or not, but i think it is

yeah it's horrifying to just have these things suddenly appear
makes the game borderline playable tho tbh
