I don't understand how to win

No that's not it actually. You still get mission complete even if you die. They probably didn't get to the extraction zone in time which resulted in mission failed. Happened to us as well. Don't forget to stand inside the circle during extraction or it doesn't count.

so a friend and i went through the process of 2 manning a playthrough, we collected all 10 samples, and both of us were uninjured and uninfected though we had both died 1 time each, but by the end we were both there and well.

We had just vaxed before calling in the heli at drilling rig, we go to the pad, both standing in the ring, nothing attacks us. the heli arives, we both still fail the mission. WHAT AM I MISSING? 10 samples, 2 out of 2 players alive and clean.

This has got to be bugged.

Same issue with our group. The points awarded SEEM like the points for a win, but we get the "mission failed" screen and do not get the achievement for the corresponding win.

best i cant tell you get 500 for a complete 10/10 failed mission, and i think something like 2000 for a success, might not be 100% on the numbers though.

Don't die, don't be infected, be at the evac-pad when the evac arrives. In the past I've gotten as much as 9000+ points for a 10-document victory run. I've heard of people getting more. But usually I lose, or die, or I'm infected on the pad and just 'Fail' so I only get around 500.

There must be something else causing this because I died once, was respawned, and had a mission success screen (all in green) so it doesn't seem like having died is the only reason, if at all.

The other time we did 10/10, I did not die once, was not infected, chill mission overall, everyone on heli pad and everyone got "mission failed".

Just got this last night. 8/10 CM and failed. Everyone alive, no infected, and failed with zero achievements. I'll try again tonight.
