Nuclear Nightmare
Insta-death and reinforcements
If you kill any of the monster/infected while its close to you, the head parasite instantaneously kills you with a locked in animation. If you are playing like a normal person you won't die from anything but infection so reinforcement as it is now is lame and makes the game less fun when playing with friends as one or two people are forced to sit for up to 40 minutes just spectating

Agreed, this is my biggest problem with the game.

If you kill the infected with a headshot the head tends to die as well. Fire and explosions can also kill the head before spawning. Killing the infected monsters by any other means will spawn a head that will kill you, but this danger is based on proximity. If you are a distance away and do not approach the head then it can not and will not jump on you. For the most part, the head will flee and not run towards you. When fighting an infected monster make sure you have plenty of distance between you and them.

For when you can't maintain that distance (Underground Silo, or getting ambushed from around a corner or through a closed door) if your weapon allows it you should sprint backwards (Shift+S) and fire as you flee. This will help keep the monsters away from you as you fight back. Rifles don't work well, as the round tends to fire up and to the right, nearly to the edge of the screen, and pistols are very similar. Shotguns and Flamethrowers are best for fighting monsters that get too close.

The head spider is a big problem once any monster gets close, and yes, you're right, it will usually result in someone getting instantly infected if it drops from a dying monster within close proximity. Hopefully the devs will find a way to balance that out (or, perhaps, add more ways to kill the head pre-death of the host, or perhaps just go full tilt and make a 'pregnant' variant that spawns a swarm of the unholy creatures on death to kill everyone and not just one player that happened to be too close).
