Is it a bug that the white lady on the left always kills you, even when you "don't look"? Or do I just don't get the puzzle right?

That White lady isn't a bug You HAVE to look at it.

Edit: I didn't see the display saying "Don't Look", That is indeed a bug.

That's a bug. The Japanese line says "目を離すな" which means "Do not look away". But the English line says "Do Not Look".

The game doesn't remember changes to the graphics options. When I change the preset from high to epic, it goes back to high after I restart.

you need to keep looking the white lady on left, and open the door fast enough, otherwise she will catch up on you.
As for the lady in the middle of the way, you need to run when the light is on. She will move when light turns off.

画面がずっと横方向に回転し続けていて、まともにプレイできません。
回転のせいで画面酔いするので、オプションで横方向の感度を下げると回転速度が落ちますが、その代わりに方向転換速度も遅くなるので、素早く方向転換して逃げないといけない異変が来ると、もう先に進めないのでゲームを閉じるかセーブデータを消して最初からやり直さないといけません。
全然進めないので、何とかしてほしいです。

On new game+ at the end of your final anomaly (Exit 8), if you sit on the seat for a while and do not exit the train you hear footsteps with an unlocking sound to the train door. You can then proceed through the door essentially passing the entire map indefinitely. Every second pass has the doors open and when you walk out into the void, you fall out and restart back at exit 8. Is this a glitch or some sort of Easter egg?

I have problems with the game. It crashes every time I run into an anomaly instead of restarting. I have to stop it through Steam or task manager. Is there something I can do to fix that?

申し訳ありませんが、カメラが勝手に回転し続けるのはおそらく環境によるものだと思われます。
別のゲームのページですが、PCにUSBゲームパッドが刺さっていたり、ゲームパッドの設定などによって発生する事があるようです。
ご参考になれば幸いです。

The hair on the guy sitting on the train seems to glitch white around the edges.

If you get off at the underground platform without internet it will just hang as you walk off the train, endless foot step noises but nothing else happens
