Have you sent in a ticket to 2K Support?
Do you have anything else connected via USB?
Are you using any 3rd party controller software?
I have an original Xbox One Controller, without 3rd party controllers software.
I have tried today plugging ONLY the controller, without keyboard or mouse, and it stills shows the generic buttons (1234), both with Steam Input enabled or disabled. Via USB I have an external hard drive connected only.
Steam Input correctly recognises the controller as a xbox controller, so, again, it's just an issue with nba 2k25.
Sent the ticket to 2K Support, so far, no helpful. To try the typical things reinstalling drivers and all of that stuff...

I tried connecting and disconnecting the controller ALREADY inside the game, and it doesn't work. The PC recognize it as a Xbox Controller, it just don't work in game.

The most weird thing is that I tried to play a game, and when I get the ball, the player just shot to basket, without inputs.
I don't know... too bad it was a gift because I couldn't even ask for a refund.
I said as you commented, paying close attention to that nº1 controller, and here are my findings:

As you see, it looks like there are 2 controllers, 2 CPUs, the Player 1 (Xbox Controller), and a Player 9 (Unknown).
I tried to uninstall and disable ALL HID, and the ones you see on next image remaining are the Keyboard and Mouse.

I tried disconnecting both from the PC, and yet, Player 9 remained there. I tried to disconnect also my external hard drive, yet, Player 9 still there.
It looks like somehow, NBA 2k25 detects a ghost controller in my computer, but I can't imagine what it is. Maybe it's the speakers, the monitor... who knows.
Let me know if something rings on your side, it's already making me crazy
