You do lose karma only if you kill enemies that aren't hostile and are injured or surrendered.
If I recall the specific location, yes, but if you do the impact is not on karma, rather additional hostiles appear. There is also at least one place where an individual surrenders (do not kill him).
Ok, I found the list of + or - moral, trying again.
I finally figured it out. You don't "follow" them, you basically have to lead them. After the first check, you turn right and work your way over toward a breaker panel, climb a ladder and move across the room above #2, then they move there and do their check. Then they start moving to the 3rd and you have to climb down and jump into the sewer duct to move around to #3. #4 Automatically triggered while I was still in the sewer duct. BTW, each service check is at a red pressure wheel that looks like the wheels you turn to open some doors, they are spread out across the room. Hope this helps, was frustrating.
