Where I'm the map do you get the Orc berserker?

I know you have to complete some quests in an area to do it. But can someone tell me which area?

The cave at Caradhras where you land at the flight point, there is a troll in there which you need to finish some quests for.

You also need to finish a level, but I forget which one.

It's the one where you play a flash back to a battle against Azog as a young Thorin. I can't remember the actual name right off but it's the third level you play in story mode. The banner has Azog's face on it when looking at it on the map.
