It's heavily implied through the game over and the "Deicide" achievement that God himself has died from a game of Buckshot Roulette against the Dealer. As for who initiated the challenge and when, probably just up for interpretation.

Assuming that god took form of a human and somehow died from a few buckshot rounds. or maybe the dealer coerced him to play with him, it would actually make sense.

Which would also explain why the page you can find that God signed is in blood, so that would mean that he died.

Or that's the dealer's blood and God walked away with the money after a billion double or nothings and cheated the leader board like a bunch of other people. The dealer also being an immortal deity.
