Cigs, beer and pills are my least priority. I never smoke cigs at full health. Why should I?
you also get turns where you had zero chance to win because of item's given and the dealers held items. I've played games where dealer had 3 Adrenelins.

being first doesnt mean you'll win.
I don't expect to win every round. it's just unfun when he pulls an adren and you have to use your good items because he WILL take them on his round.
More so think of this thematically, i dont think the dev expected this game to be more popular past his initial horror audience (perhaps), this is a game that is a representation of gambling addiction and a dystopian setting where only the thrill of losing your life saves you from numbness.
Its just ♥♥♥♥ you have to deal with and plan around and even then you are going to end up losing but it is what it is. My best advice is to learn the dealer's patterns, what items he prioritizes first so you can plan around what items to leave and what needs to be used immediately, usually dont leave yourself any glasses, knifes or cigs, usually in that order as he prioritizes glass a lot. Also use cellphones immediately if you have them, they almost always offer you different info each time you use it, also avoid using pills as much as possible, those are good fodder when left and have a good chance to do 1 damage and could be the difference between going into next mag or winning on that one.
Lastly, some personal annecdote that just happened yesterday: on one of the first 3 rounds the round was about 4 charges, i managed to leave him at 1, however we had both lots of items and i forgot 1 magnifying glass and a pack of cigs. Long story short, he got into full health and i was left without items at the end of the magazine, so basically a full restart with him having a full table. Later on in that same game i finally got the 1M achievement, so that horrible round was frustrating yet it didn't matter at the end of it. What DID make the difference, was the horrible amount of luck i had when i had like 3 rounds in a row that were 2 lifes each and i got luck on my side, both for shots and some items helped too. So try to not stress about it, you can have amazing rounds back and forth for like 10 or so rounds? you are just one round away from losing
