Betrayer is similar as well but sadly it's no longer for sale on Steam...

I just thought of another one : Clive Barker's Undying.
It's available for sale on GoG but it's a real pain to run in widescreen and it's impossible to run in resolutions higher than 1920x1080.

I've been hunting for quality horror-themed FPS' in gothic or old-timey settings for years and these 6 games (including Nosferatu) are just about all there is.

Thanks! Really appriciate your research and Insaniacs too. I put all of them into my wishlist, also seek out the trick how to get Betrayer.
And I mean this THANKS.
I asked for "similar games" on different occasions before and the responses were horrible :D f.e. the gem that is RTS Original WAR.

Yeah I'm not a fan either but I just wanted to give you all the options possible since the game is now abandonware and the devs aren't making money with it anymore regardless of where you buy it. The game was self-published by Blackpowder Games which is now defunct.

I saw a few mins of gameplay from Civvie 11 and thought I should play the game before seeing it. The very little I saw gave me a They Hunger vibe, a Half-Life 1 mod with three episodes (Came out in '99, '00 and '01 respectively, close to this game's year). You can also play them cooperatively in Sven Co-op (Though I suggest getting friends instead of randoms as they will rush it)

Super late but Vampire: The Masquerade - Bloodlines is freaking awesome. Lots of exploration and a very effective unofficial patch that makes it runs much better on modern systems

The remade/re-imagined Waxwork is very similar to a point. Still lightly janky, but an improvement of the original. [As of this post, the original is 75% off on GOG...]
