How much do you lose when you die?
Title.
How punishing is it if you fail to extract from a mission?

Loot lost and gear you can get back, if you go there again.
You lose what is on your character. if you go back to the same location you can loot your corpse (from what I understand) and get [something] back. Was watching someone play yesterday on twitch, and they died, went back, and looted some stuff from where they had died.

I don't own the game, so I can not give more details than that. I don't about any upgraded items, etc you take with you.

If you die, You can retrieve your Rig parts, Weapons, and Weapon attachments from what I've been able to get back. Other stuff is gone.

You lose your loot (items in your containers/cages/racks) You lose your guns+parts and any reinforced/upgraded rig parts, but not the rig itself.

When you go back and loot your gravestone, you can recover your weapons+parts, and Rig upgrades. However your loot is lost for good.

I am not sure on this last bit, but I have also heard that dying in mission after recovering your gravestone will not recover anything. and you will lose it for good.

So far there's almost nothing you can't easily get back once you've unlocked all the vendors and everything you can lose is generally cheap to buy (like you can get enough for 4-5 rifles from a single run with a basic rig). The anti-tank rifle is the only exception that immediately comes to mind. I guess it's technically possible to lose more ammo in one go than the vendors stock. You will keep your rig (but not any customized parts you've added) and that's by far the most important piece of gear in the game.

If you die due to a buggey landscape... it says 'you are down' and if you are solo there's no way of getting back up you can't get your stuff becuase as soon as you go to that area you go down again. what would be good is a recovery bot you can carry which will return with your backpack if you go down.

This is slightly misleading.

I won't deny that there are bugs in the game (EA)
But I play solo, and I'm far from a pro-gamer these days, and I can easily retrieve my rig. You don't necessarily go down again.

I would say Souls-like game are far more punishing than this is.

A recovery bot is good idea though, like a one time revive if you're solo.

When you die, you lose every items you brought and loot you've found in that raid definitelly

What remains is a tombstone with your weapon/upgraded rig. If you touch the tomb and manage to extract, thoses things will get sent back to your inventory.

If you touch the stone and die before extracting, your actual gear will get added to the new tombstone. So if you touch the stone and extract, you'll get back the gear from your 2 raids.

If you die without touching the stone, your previous upgraded rig & weapons are definitelly gone.

If you extract without touching the stone, your previous rig & weapons are definitelly gone.

Your character's prestige and XP are kept even if you die, as well as the base layout of the rig you selected

No not true, I can vouch for that if you do grab your gravestone and still end up failing the extract, as long as you go to the same exact map. You can still get your rig parts, weapons, and weapon parts. It's a timed mechanic for how long the gravestone sticks around. I don't know the time limit as I bum rush my gravestone.

You just lose what is on you then. So rig, weapons, parts, loot. You can recover the Rig and weapon parts, nothing else though. Just as has been said, if you die in a map, you best bet is to run a fast as hell bumrush to get your ♥♥♥♥ back to go back to your normal load out. Or if you have the cash, just laugh and reload it.
