I had already suggested this a few years ago!
It always makes me sad too.
The only thing I can offer you, is my sound mod.
It replaces that horrible sound when a leader dies!

One thing you could do is create a "phantom nation" - similar to when you set up a mercenary enclave. Then when you engage in "diplomacy" with the nation, it would show you the deceased leader's name and visage along with whatever "remembery" text were associated with it.
In Stellaris\common\on_action\00_on_action.txt there's a little hook where an event can be fired on leader death. In my (pre-Paragons) file it looks like this:
So you could easily custom code an event that fires at the right moment. I'm not sure if the leader's info gets wiped as soon as they die. Though again, you could handle that by generating their "memorial nation" as a silent artifact when they're first created. Then when the death event fires, if they're not worth remembering, their little memoriam nation would be deleted. And if they were worth remembering, they'd get their little memorial page.
The game simply doesn't track a lot of that data, which is one of my frustrations with it. When I was first learning the ropes, it'd drive me crazy that entire species would go extinct before I'd encountered them, and there would be no way to retrace who they'd been or what had happened to them.
EUIV handles that a lot better. Each province has a "history" button that tells you when it's traded hands or when a significant person was spawned and each nation has "annals" that list the major incidents during a ruler's reign. It's not necessarily gripping reading, but I really enjoy that feature of that game and wish there were something like it in this one.
We really need to have an Archieve of all the Leaders which Build that Empire.
Including maybe some Info on their Bigger Achievements as well as Cause of Death and from when to when they Server the Empire.
Not just to avoid Governors just suddenly Dissappearing and Colonies being without Governor for Decades before someone notices.
But also just because its part of the Immersion for the Game.
And very often throughout a Game You have certain Leaders which are really carrying the Empire.
It should turn the leader death sound to the Lego Star Wars Yoda death scream. XD
It's crazy to me that the game has "memorialist" civics and "death chronicler" jobs, alien citadels out the wazoo, and yet... no place where you can go and access such chronicles.
I totally think there should be a Memorialist Citadel that just sells intel on vanquished countries and extinct species.
