Known to cause desync in the latest patches (still work rock solid in single player)
If no mods involved (aka you playing vanilla) then desync can be caused by a variety of factors.
Mostly network latency that causes packet loss. If you or your friend internet connection has a suboptimal latency, desync is very common. This is most likely on the player that host the game.
Another cause can be third party software, background applications like firewall or AV , can interfere with the communication, but this is less likely, still something to keep in mind, try to whitelist the game on both you and your friend AV/Firewall.
Also worth for both of you to verify game files integrity to make sure no one of you has corrupted files.
Usually happens when the hardware performance of client & host are too far apart. (for example, a 7800x3d vs 3700x CPU)
This can be prevented by the player with the weakest hardware hosting the game.
Or by setting the ingame time to very slow. (But it can still happen that the hardware performance is still too far apart).
If this only happens once, save the game and host a new lobby.
And you can try changing the host.
If this happens more often, then well, I don't know. There are lots of forum posts, some of which have been happening for years. The cause is unclear.
